I EKUZ – European Health Insurance Card Can be covered by the voluntary health insurance – a written application with NFZ (National Health Fund)

Documents required:
1. passport
2. registration of residence
3. certificate from dean’s office conforming student status
II – ZUS – Social Insurance Instiutions
Document required: signed contract with NFZ

You will receive ZUS ZZA – document confirmation of application to social insurance.
III – You will pay social contribution – 55,80 PLN per month Go to the Student Affairs Department

Documents required:
1. Signed contract with NFZ
2. ZUS ZZA
3. Pole’s Card Then The

University will cover your social contributes
