List of Documents for International Applicants

  1. Printed online application form
    • the candidate confirms the accuracy of the data by signing the form personally.
  2. Copy of the secondary school leaving certificate
    • together with the original for verification by the university;
    • the document must entitle the holder to apply for higher education in the country where it was issued.
  3. Foreign education documents
    • must be provided with an apostille or be legalized, depending on the country of issuance.
  4. Proof of language proficiency
    • candidates required to document language competence must submit a certificate confirming proficiency in the language of instruction at minimum B2 level (ATTACHMENT).
  5. Consent for minor applicants
    • applicants under 18 must provide consent from parents or legal guardians to undertake studies

    Accepted forms of signature certification (one of the following):

    • notarized signatures, or
    • certification by a Polish diplomatic or consular mission, or
    • certification by an authorized official in the applicant’s home country (e.g., office, notary), or
    • signatures made in the presence of a member of the university admissions committee.
  6. Translation requirement:
    • if the document is in a foreign language, a Polish translation must be attached.
  7. Template:
    • the consent form template is available on the university website. (ATTACHMENT)
  8. Translations of documents
    • all documents issued in a language other than Polish must be translated into Polish by a sworn translator.
  9. Information from the Director of NAWA (Polish National Agency for Academic Exchange)
    • required for candidates whose certificates were issued in countries outside the EU/EFTA/OECD or in countries without agreements on mutual recognition of education.
    • not required for candidates holding certificates along with a decision issued by the Education Office in Poland before July 1, 2025.
  10. Photographs (two versions of the same photo):
    • one printed photo (meeting ID card requirements),
    • one digital photo for the application system: JPG format, 2 cm × 2.5 cm, 236 × 295 pixels.
  11. Health insurance policy (required after the admission process):
    • covering medical treatment and accident insurance,
    • valid for at least the first year of studies in Poland;
    • or a European Health Insurance Card (EHIC), or a declaration of voluntary insurance with the National Health Fund. Regarding the following programmes: biology, biotechnology, chemistry, dietetics, pharmacy, physiotherapy, physics, technical physics, medical physics, medicine, cosmetology, nursing, midwifery, diagnostic systems in medicine, emergency medical services.
